I seem to lose the odd companion now and then - usually Khelgar - and just
now again, after restarting and during different scenarios (my heroes) and I
think it more often happens in the transition time from scene I to scene
II - but I lost Kelgar in the tunnels on the way to deal with Zearrie too.
Anybody else have this happen? Why?

There are several threads about this over at Game Banshee - as Kharsis says,
one solution is to play from an earlier save because it doesn't always
happen at the same place and time. But there's another fix which involves
replacing the character file I think - let me see if I can find the thread.

Have a look here:

and, more importantly, here:

thanks for the comments folks - I see that it's a common problem now
I have been able to go back to saved games ok, but it has been a real drag
to find I've lost someone after a long cutscene(s) like the change from Act
I to Act II and have to listen to them all over again and again.
Dave.

I lost the tiefling on one transition

All I have been able to find is tat it is a random bug. Only solution
is to go to a preveious save with that character.
