ice cold balls

In the Taito Ice cold beer game:

All of my games seem to default to 5 balls per game instead of 3 balls.
The only thing I switched was the dip switch settings to give an extra
ball at the 4000 score. Even If I switch them back to the factory
default or "no extra ball" I am still seeing a 5 ball game and not a 3
ball game. Furthermore the extra ball score is not giving me a 6th ball
at the indicated score level anyway.

I can't find it in the manual anywhere that says there should be 3
balls versus 5. Do you know what the right answer is and how to go
about fixing it?

One more thing. Where is a good place to get light bulbs? I need the
coin door lights.
