I've been fooling around with an enhanced Q1 engine called Darkplaces:

I've always found lots of servers with a decent number of people
playing. They're all overseas so pings would affect an aussie like me,
but given the right client, yes there are still players.

The All-Seeing Eye reports 73 players in 20 (of 618 total available)
QuakeWorld servers at this particular moment, from all over the world. Not
much compared to the old days, but not entirely dead, either.

A good way to get started is with the fQuake all-in-one Quake installer
(successor to eQuake), which includes the standard QW clients in current use
(FuhQuake, ezQuake, etc.), ASE, and numerous other utilities/add-ons/maps,
etc. - really everything you need.

To get an idea of how Quake is still being played (at least in Europe), see
http://quakeworld.nu/
