Of course I should have checked this out before replying from memory.

Anyway, what kills the opening video is the ESCape key, not the Enter
key. And once in the settings, you have a lot more options to choose
from than 640 by 480, all of which are good. The main point remains,
though, that after a first installation of CFS2, it initially does want
to run in 640 by 480, and that can cause problems. Like a black screen
while the audio plays, for example.
If, as you seem to say, your choppiness doesn't occur until well into
the game, I'm guessing that might be a problem with your video codecs,
but that's just a guess. Have you tried locking your framerate to
something reasonable like, say, 35? Sorry to have provided bad info in
first reply. Microsoft didn't release any patches for this game.
