Nope, they never run out. I personally enjoy doing the "rescue
kidnapped soldiers from Nemesis" missions that you get from that guy
near the eastern exit in Zulu. Nemesis are reasonably easy enemies
for a Fire Tanker and the missions are usually easy to reach. (To my
amusement, one of them is *within the Zulu fortification itself.* No
wonder the expeditionary force keeps misplacing its patrols!) I also
like keeping a Kora Fruit mission on retainer so I can always
restock--though since the Kora fruit nerf, that's a bit less
attractive anymore.

I've never had that happen to me, though it sounds like it might be
related to the bug I'm seeing in the PVP zones, which exhibit the same
behavior to my heroes but not my villains.

You still get debt, 110K a pop if you die outside of a mission, 55K if
you die in. You'll be Exalted before you know it. (I wish there were
debt badges after that; Exalted is entirely too easy to get for it to
be the final stop.)
As to whether XP is still being tracked: I doubt it. XP gained after
hitting 40 on CoV was not tracked; you started right at 40.0 when I7
came in, and that was when they KNEW they were going to be adding 10
more levels immediately. There's no indication that we'll be seeing
levels 51-60 anytime soon; up until relatively recently they had no
intention of *ever* adding them, and I still think they aren't really
considering it seriously.
Level 50 is a great time to do the things you've always wanted to do.
Take whatever crazy risks you want, join the most incompetent pick-up
groups. All you'll get is debt and it's not like the debt will do
anything except prevent you from earning Influence when you're
exemplared down until it's cleared out.
