Old and busted version:

Nevinyrral's Disk -- 4
Artifact
Nevinyrral's Disk comes into play tapped.
1, T: Destroy all artifacts, creatures, and enchantments.

False Demise -- 2U
Enchantment -- Aura
Enchant Creature
When enchanted creature is put into a graveyard, return that creature
to play under your control.

New hotness version:

Magus of the Disk -- 2WW
Creature -- Human Wizard
Magus of the Disk comes into play tapped.
1, T: Destroy all artifacts, creatures, and enchantments

Fool's Demise -- 4U
Enchantment -- Aura
Enchant Creature
When enchanted creature is put into a graveyard, return that creature
to play under your control.
When Fool's Demise is put into a graveyard from play, return Fool's
Demise to its owner's hand.

Question: I have a Demise (either one) enchanting a creature in play.
I set off a Disk (either one). The end result: Do I have a creature
under my control on an otherwise creature, enchantment, and
artifact-free board?

If the Demise and the creature go to the graveyard at the same time (which
normally doesn't happen, but can with the Disk or some other things), the
Demise triggers, yes. Its triggered ability is a leaves-play one, so looks
back to the game state just BEFORE the creature goes to the graveyard to
see if it exists so it can trigger. And lo, it does.

Twice the triggering fun; if this Demise goes to a graveyard at the same time
its enchantee does, both its abilities trigger, same reason as above
(essentially).

Well, except for the indestructible artifacts (or creatures or enchantments),
and ones that got regenerated, yes you do.

Yes, why wouldn't you?

410.10d Normally, objects that exist immediately after an event are
checked to see if the event matched any trigger conditions. Continuous
effects that exist at that time are used to determine what the trigger
conditions are and what the objects involved in the event look like.
However, some triggered abilities must be treated specially because the
object with the ability may no longer be in play, no longer be in a zone
visible to all players, or no longer be controlled by the appropriate
player. The game has to "look back in time" to determine if these
abilities trigger. Abilities that trigger specifically when an object
leaves play, when an object leaves any visible zone for a hidden one, or
when a player loses control of an object will trigger based on their
existence, and the appearance of objects, prior to the event rather than
afterward.
Example: Two creatures are in play along with an artifact that has the
ability "Whenever a creature is put into a graveyard from play, you gain
1 life." Someone plays a spell that destroys all artifacts, creatures,
and enchantments. The artifact's ability triggers twice, even though the
artifact goes to its owner's graveyard at the same time as the
creatures.
